5 AI signals every CIO should be watching right now

Across analyst research and hands-on enterprise deployments, a consistent pattern is emerging. The most important signals CIOs should watch over the next several months are not new AI features or model benchmarks but behavioral, organizational, and governance signals that quietly indicate when AI has crossed from tool to actor inside the enterprise.

Forrester predicts that by the end of 2026, CIOs will be forced to decide how far workflows can operate without humans. The challenge is that many organizations are already drifting toward autonomy without explicitly acknowledging that decision.

Based on interviews with Forrester VP and research director Linda Ivy-Rosser, and IT leaders from Trimble, Cisco, and Phison Electronics Corp., five signals stand out. Each offers CIOs an early warning system, not just of technological change, but of operating-model transformation already underway.

1. In workflow and autonomy: when AI stops assisting and starts acting

The earliest and most consequential signal is deceptively simple: AI systems begin taking actions without being explicitly invoked by humans. At tech company Trimble, Aviad Almagor, VP of technology innovation, describes the moment autonomy quietly arrives. “The line is crossed when AI stops answering questions and starts taking actions,” he says. In early phases, systems may recommend next steps. But once AI starts executing those steps, the workflow has fundamentally changed.

Another telltale sign is behavioral. Almagor says teams stop asking, “What prompt did you use?” and start asking, “Why did the system decide to do that?” That shift indicates the AI is no longer perceived as a tool but as a decision-making participant.

Cisco principal engineer Nik Kale sees the same pattern in large enterprises deploying AI assistants at scale. Initially, humans review AI outputs before they reach customers. Over time, as confidence grows, that review becomes a rubber stamp. Eventually, humans are only involved after something goes wrong. “The moment humans move from the decision loop to the post-mortem loop, you’ve crossed the threshold,” he says.

This signal means the organization has shifted from assistive AI to agentic AI, often without a formal decision. CIOs who miss this moment risk managing autonomy reactively instead of intentionally.

2. In governance and risk: when control fades faster than accountability

One of the clearest red flags appears when audit trails explain what happened, but not why. Almagor warns that many organizations can reconstruct actions but not reasoning. “If no one owns the decision and AI made it, governance is already behind,” he says.

Forrester’s Ivy-Rosser sees this most often when AI is deployed to fix messy, non-standardized processes during crisis conditions. “CIOs pick the path of least resistance,” she says, bypassing the hard pre-work of defining decision rights, escalation models, and orchestration blueprints. The result is cascading operational risk, not because AI fails, but because governance never caught up.

Another under-appreciated sign is rollback difficulty. Kale advises CIOs to watch how expensive reversibility becomes. When undoing an automated action requires coordination across multiple systems or teams, that’s when autonomy has expanded beyond its intent. “Autonomy should be granted in proportion to reversibility and containment,” he says, pointing out that confidence in the model is a smaller factor.

This signal shows that autonomy has outpaced governance. Once reversibility becomes costly and accountability diffuses, organizations then operate beyond their risk tolerance whether they realize it or not.

3. In operating models: when work reorganizes itself around outcomes

Another signal shows up not in dashboards, but in how work is described. At Trimble, Almagor points to a shift from role-based execution to outcome-driven workflows. Instead of siloed AI tools supporting schedulers, field operators, or planners independently, agentic systems now monitor end-to-end conditions and adjust plans continuously. “When work is organized around outcomes instead of roles, the operating model has changed,” he says.

Forrester sees similar patterns across industries. Ivy-Rosser notes that many organizations have handed over process complexity to vendors through managed services without shifting to outcome-driven contracts. “Vendors end up making strategic decisions because the enterprise never clarified where utility ends and competitive advantage begins,” she says.

A related signal appears when CIOs are asked to intervene after AI initiatives fail. Forrester predicts that a significant number of CIOs will be called on to bail out business-led AI deployments that lacked governance and shared accountability. This is less a failure of technology than of operating-model alignment.

This signal suggests that AI is reshaping how value is created and delivered. CIOs who still frame AI as a productivity overlay risk missing deeper structural change.

4. In culture and behavior: when humans change faster or slower than systems

Several of the strongest indicators are cultural. Organizations ready for higher autonomy display comfort with probabilistic outcomes. Almagor emphasizes that successful teams don’t expect deterministic answers from AI systems. They treat uncertainty as an input, not a failure, and design thresholds and human-in-the-loop mechanisms accordingly. “Autonomy fails not when systems are uncertain but when organizations are,” he says.

Conversely, over-trust is another warning sign. In construction and transportation contexts, Almagor has seen AI systems continue confidently despite missing or conflicting data. The danger escalates when humans stop questioning outputs because they claim automation has always worked before.

Kale describes a similar phenomenon at scale. Humans disengage once AI performance stabilizes, even as the blast radius of decisions grows. This quiet erosion of vigilance often precedes governance crises.

This signal reveals whether the organization can absorb autonomy responsibly. Technical readiness without behavioral readiness is a leading indicator of failure.

5. In technology and infrastructure: when constraints move below the application layer

Sebastien Jean, CTO of Phison Electronics, highlights infrastructure bottlenecks that quietly determine success or failure: memory shortages, data locality, and latency tolerance. “If a system takes 17 minutes instead of seven seconds, people will simply walk away,” he says. These constraints shape adoption more than algorithmic sophistication.

As AI initiatives move from POC to production, he adds, many organizations assume that scaling requires running the full version of a system everywhere — larger models, more memory, higher bandwidth, and premium infrastructure tiers. In practice, Jean says, that assumption often goes untested.

Instead, he describes a more empirical approach that some teams are beginning to use, which is deliberately running a reduced version of the system alongside the full one, and comparing outcomes. “You can take one version of the system, reduce the resources and model size, or simplify the pipeline, and then measure whether the business result actually changes,” he says. In many cases, organizations discover that performance differences are marginal or invisible to users while infrastructure costs drop dramatically.

The key signal for CIOs, he noted, is when decision quality, user behavior, or downstream outcomes remain stable despite the reduction. That stability indicates the organization has been overpaying for capacity it doesn’t need.

Cost optimization becomes a signal of maturity. Organizations that can safely degrade, compare, and validate outcomes are no longer guessing where their AI spend delivers value. They’re measuring it and using that evidence to guide both architecture and governance decisions.

How to act on these signals before they act on you

Across all four interviews, the one consistent message is that these signals aren’t warnings of future change. They’re evidence that change is already underway. So the CIO’s job is to institutionalize how the organization responds once they appear.

The first concrete step is to formalize signal detection. CIOs should stop relying on ad-hoc anecdotes (something feels different) and instead build explicit review moments into governance forums. That means regularly asking questions such as which systems are initiating actions without prompts, where humans are only involved after the fact, and which decisions are hard to reverse. As Almagor at Trimble says, autonomy often sneaks in through convenience. CIOs need periodic, intentional reviews of where that convenience has accumulated into control shifts.

CIOs should also pull governance forward, not layer it on later. Forrester emphasizes that retrofitting controls after deployment is often more disruptive than slowing down early. Ivy-Rosser stresses the importance of decision rights, escalation paths, and orchestration blueprints before agents operate end-to-end.

At Cisco, Kale says that instead of framing autonomy purely as a design choice, it’s better to look at a behavioral signal. In large-scale deployments, he says, the real threshold is crossed the moment humans stop being in the decision loop, and start being in the post-mortem loop. At that point, AI has effectively become an actor rather than an assistant, often without an explicit decision by leadership.

Once signals indicate autonomy has past that threshold, CIOs must reset operating and accountability models. And when humans become exception handlers, and AI spans workflows, shared accountability is no longer optional. CIOs should convene COOs, CHROs, legal, and business leaders to explicitly define who owns intent, execution, and outcomes. As Kale observes, AI doesn’t remove accountability, it forces enterprises to finally clarify it.

Finally, CIOs should treat culture as an operational control. Organizations that handle probabilistic outcomes well and challenge automated decisions are better prepared for autonomy than those chasing deterministic certainty. That may require retraining managers as supervisors of digital workers, not just consumers of tools — a shift Jean of Phison likens to managing skilled junior employees rather than software.

So spot the signal, name the shift, and act deliberately. CIOs who do will shape autonomy on their terms rather than inherit it by accident.
